Instructions to book appointment with Central Registration number (revisit patients of Hospital)
Hospital has started Online Provisional Registration Facility. Those who are using this facility may avoid standing in long queues.
Enter your CR No. — OTP will be sent automatically after entering the 15-digit CR No.
Select the department where you want to revisit or select a new department/hospital you would like to visit.
The provisional registration is valid for 30 days from the date of provisional registration.
On the day of the tentative time slot booked, show your CR No./provisional registration no. at the Registration Counter/OPD bay during hospital OPD timings.
You can avail this facility up to 30 days in advance from the date of visit.
If you want to book provisional registration for the next day or upcoming days, a maximum of 30 days advance booking is allowed.
Please bring all your old OPD cards/reports when you visit the Hospital.
